1. Why is 3M issuing this Reminder Notice?

On August 30, 2016, 3M issued a Recall Notice of the original Lad-Saf sleeve. We have worked diligently over the last year to withdraw as many original Lad-Saf sleeves from the field as possible.

We are now going out to the field a second time in an effort to ensure that end users are aware of the recall and to encourage end users to take their original Lad-Saf sleeves out of service and to return them to 3M Fall Protection.

We are offering a free X2, X3, X3+ or a cash re-imbursement of $200 USD for each original Lad-Saf sleeve returned to 3M Fall Protection.

2. Why did 3M issue the Recall Notice?

After a review of reported incidents in the field, we identified potential misuse scenarios that could compromise product performance and result in serious injury or death in the event of a fall.

3. How do I obtain a replacement sleeve or the $200 USD cash reimbursement?

All you need to do is access the dedicated website at www.LadSafRecall.com and enter the required information. If you do not want a replacement sleeve shipped to you, then we will reimburse you $200 USD per sleeve provided you return your original Lad-Saf sleeve(s) to us.

4. This is the first time I have heard about this recall and I don’t want to interrupt my site operations. What can you do to assist me with this?

There is a hot button on the “Contact Us” page of the dedicated Lad-Saf Recall website that will take you to a regional contact list. Please call or email the regional contact identified and they will assist in expediting the replacement sleeve to you.
